Client Intake Form

Welcome! I’m so delighted that you’re interested in working with me. In order to best assist you, I have created a form below to gather any key requirements. These details will help me determine if the collaboration is a good fit, create a proposal, and prepare for any meetings. Additionally, if you can provide any details on success metrics and/or your target audience, I can truly create a proposal that helps you achieve your goals. Expect the form to take about 5-10 minutes; don't feel the need to spend too much time on it.

Now booking for due dates of November 2021 and beyond. Projects may be accepted for October 2021 deadlines with an additional 20%-25% rush fee.
